With this installment there are 5 harnesses and 23 items (counting the 4 new items) that alter and enhance the character's attributes.
As well as that, there are items you can collect during a battle that take effect immediately, these are listed in the manual but they are listed here for convenience as well.
War God's Axe - Attack x2 for 30 seconds
War God's Armor - Defense x2 for 30 seconds
Speed Boots - Speed max for 30 seconds
Imperial Seal - Unlimited Musou Attacks for 10 seconds
Meat Bun - Life +50
Meat Bun x2 - Life +100
Meat - Life +200
Whole Chicken - Life +400
Wine - Musou Gauge recovers to Maximum Capacity
Elixer - Life and Musou recover to Maximum Capacity

This gauntlet-like piece allows the user to perform charge attacks uninterupted even while enemies are inflicting damage regardless of how strong they are.
Battle of Si Shui Gate - Dong Zhuo's side
Defeat Sun Jian before the supply depot has fallen. The item will appear in the northern passageway, east of your main camp.
Art of War

Increases the longevity of Attack, Defense, Speed, and Musou in-battle pick-up items.
Battle of Chi Bi - Cao Cao's side
Defeat Zhuge Liang as he is retreating. This can be done in a number of ways, as he retreats if the fire attack fails, and also retreats if he succeeds in making the wind blow. Once you have defeated him, the item will appear in the middle of the map, to the west slightly, on Wu's side of the boats.
Bodyguard Manual

Bodyguard becomes noticeably stronger.
Guan Yu's Escape - Guan Yu's side
Defeat every general and secure every gate. The item will appear back at your starting point.

Demon Band

New to the series, this decorative headband will lengthen the effect time of musou rage attacks.
Battle of Xi Jing - Zhang Jiao's side
In the northwestern area, save Zhang Liang by defeating the generals there, and securing the strongholds. Then kill every enemy general, including their reinforcements. The item will appear on the alter, near your starting point.
Dragon Amulet

This necklace of the Blue Dragon will increase your musou gauge stats proportionate to its level.

Elephant harness

Start the stage riding on an elephant. Found in Nanman campaigns, the massive pachyderm is unparalleled in strength, yet its bulk drastically reduces speed.
Battle of Nan Zhong - Nanman side
Occupy all neutral bases while assisting your allies, all within 5 minutes. The item will appear in the center of the lake.

Fire Arrows

Equip this and all your arrows you shoot will become projectiles of fire to inflict more damage upon the target.
Battle of Ji Province - Yuan Shao's side
Defeat Zhang Bao and Zhang Liang, then defeat Pei Yuan Shao to get the Fire Arrows.

Green Scroll

This scroll will increase attack strength at the cost of decreasing defense strength.
Battle of Jie Ting - Shu side
Protect Ma Su by defeating Cao Zhen, Sima Zhao, and Zhang He. Head over to Xu Zhu when he appears and defeat him. Then defeat Sima Yi. The item will appear around the southern area.
Herbal Remedy

A bag of supplements that will increase the attack damage of charge strikes.

Hex Mark harness

Start the stage with Liu Bei's steed Hex Mark. This dark gray horse has been rumored to boost luck while riding the horse, otherwise has no unique effect.
Battle of Cheng Du - Liu Bei's side
Defeat Zhang Ren before he kills Pang Tong in his ambush. The easiest way to go about obtaining this item is to simply be Pang Tong and defeat Zhang Ren yourself. The item will appear smack in the center of the bamboo forest.

Destroy all juggernauts within the northern area in 5 minutes time. The item will appear to the west, near the exploding urns.

Start the stage with the fastest steed among horses, Red Hare. Bright red horse with elaborate ornamentation, its superior attack and speed is what earn Red Hare the title of being "worth a thousand horses."
Battle of Chang Shan - Yuam Shao's Forces
Defeat Guan Yu, Liu Bei, and Zhang Fei all within a minute from each other. The item will appear northwest, outside of the castle. Can only be done with Player 1.

A new item, these dagger-like trinkets will increase attack strength after each 100 KOs (Every 100 KOs will give your character a permanent attack +2).
Battle of He Fei - Wu's side
Defeat Pang De and Cao Zhang. Then proceed to the southwest enemy base, where Xu Huang will arrive, and defeat him. Head over to the center area of the map, where Sun Quan resides, and defeat Zhang Liao upon his arrival. Occupy the farthest north enemy stronghold and Yu Jin should arrive, defeat him. In time Zhang Liao will appear yet again, go off and defeat him. Now go and wait near the northeast of the map for Xiahou Dun to arrive, and defeat him within one minute when he does.

Start stage on generic black horse yet retain ability to not be knocked off the horse while riding throughout the entire level. With harness equipped, any commandeered steed can be used with resistance to being dismounted.
Battle of Liang Province - Allied Forces side
Defeat Dong Zhuo's reinforcement generals, Hu Zhen and Zhang Ji, within 50 seconds of their arrival. The item will appear near the central area. Can only be done with Player 1.

Start the stage on yellow horse with yellow tail. Storm Runner increases Ride Attack +12.
Battle of Guan Du - Cao Cao's side
You must defeat Yan Liang and Wen Chou before Guan Yu does. He immediantly heads out for them, so be sure to have a fast horse. The item will appear to the northwest, in the passageway leading to the enemy's main camp.
Survival Guide

When life is critical, attack strength will double upon getting up after falling.
Battle of Hu Lao Gate - Dong Zhuo's side
Defeat Cao Cao, Gongsun Zan, Liu Bei, and Sun Jian. The item will appear near where Liu Bei and Sun Jian had started.
Tiger Amulet

This necklace of the White Tiger will increase your attack stats proportionate to its level.

A new item, with this ring you start the stage with a loyal tiger fighting by your side.
Struggle for Nan Zhong - Meng Huo's side
Defeat the three tiger trainers under King Mulu's command within a minute of the stage beginning. The item will appear inside your own camp.

A new item, this pouch of supplements allows the user to regain increments of life points in their health gauge after each 100 KOs.
Battle of He Fei Castle - Wei side
First, destroy the siege machine attacking the front gates of the castle. Eventually Lu Xun will send in another one, head over to destroy it, and take out Jiang Qin. Defeat Pan Zhang, then defeat the reinforcement general Sun Shao. The item will appear on the inside of the castle, on top of the gates.
Way of Musou

This yellow scroll allows you to do your true musou attack regardless of what health you have.
Battle of Mt. Ding Jun - Liu Bei's side
Defeat Xiahou Yuan before Cao Cao arrives. Item report will appear in the top left of the map.
Wind Scroll
(needs to be ported to the wiki!)
This blue scroll increases your attack radius.
Battle of He Fei Castle - Wu's side
Defeat Cao Ren within 5 minutes. The item will appear to the northeast of Xu Zhu.
