From Dynasty Warriors - Wiki on Neoseeker
Dynasty Warriors 3 introduces a new element to the series, items. These items boost various statistics or bestow special abilities, usually time based. Each character can hold up to 5 different items at once, so choose the ones you think will benefit you most in battle. Often a report of a valuable item or a supply captain will follow completing the right objectives. If it's a captain then you must kill them and they will drop the item.
Art of War
Increases the longevity of Attack, Defense, Speed, and Musou in-battle pick-up items.
Siege of He Fei Castle - Wei's Forces
Defeat all generals execpt Sun Quan.
Bodyguard Manual
Bodyguards becomes noticeably stronger and use a second weapon instead of just a first.
Siege of He Fei Castle - Wu Forces
Defeat Xu Zhu over by the bridge.
Buckler
This item allows you to block without being overpowered no matter what attacks come your way.
Battle of He Fei - Wu Forces
Let Taishi Ci be killed and then kill Zhang Liao.
Cavalry Armour
This reduces the amount of damage you take when you're riding a horse.

Defender
This item doubles your defense when your health gets to the red section and you get back up after being knocked down.
Battle at Wu Zhang Plains - Wei Forces
Defeat 2 of the 5 Tiger generals to obtain a shooting star event.
Dragon Amulet
This necklace of the Blue Dragon will increase your musou gauge stats proportionate to its level.

Elixer
Sack of supplements that increases the speed of musou gauge recovery.
Found in boxes and dropped by officers
Fire Arrows
Equip this and all your arrows you shoot will become projectiles of fire to inflict more damage upon the target.
Battle of Chi Bi - Wei Forces
Kill Zhou Yu before the fire attack.
Golden Harness
This harness prevents you from being knocked off when you are hit with an arrow while riding a horse.
Battle of Tong Gate - Wei Forces
Kill Pang De and Ma Dai and make Han Sui defect to Cao Cao's side. Once he's defected kill all other enemy generals and make Ma Chao retreat back into Tong Gate.
Note this item is very hard to obtain. On easy mode Pang De and Ma Dai are easier to kill but Han Sui can be killed with ease, on hard mode Han Sui will last longer but Pang De and Ma Dai have Lu Bu type power.
Hex Mark Harness
Start the stage with Liu Bei's steed Hex Mark which supposedly increases luck while riding it.
Assault on Cheng Du - Shu Forces
Kill all enemy officers except Liu Zhang.
Horned Helm
Decorative helm that enables an increase in attack power when mounted on a steed.

Huang's Bow
Imbued with the archer supremacy of its owner, this bow will increase your arrow attack damage.
Found in boxes and dropped by officers
Imperial Saddle
This saddle allows you to ride any horse regardless of what level you are.
Battle of Yi Ling - Shu Forces
Prevent the fire attack, then kill Lu Xun. A supply captain will appear at the Wu base, kill him for the item.
Note that Liu Bei will not retreat if the fire attack fails and so will be vulnerable. Either have a second person stay with him or play as him for the best chance of getting this item.
Peacock Urn
This item gives your maximum health a boost, the higher the number the bigger the effect.

Power Scroll
This yellow scroll means you will never lose a weapon deadlock, ties are still possible.
Battle of Hei Fei - Wei Forces
Kill Gan Ning when he appears just south of your main base.
Red Hare Harness
Start the stage with the fastest steed among horses, Red Hare. Bright red horse with elaborate ornamentation, its superior attack and speed is what earn Red Hare the title of being "worth a thousand horses."
Battle of Hu Lao Gate - Allied Forces
Kill Lu Bu.
Seven Star Orb
This orb increases your luck which will increase the frequency of dropped items.

Shell Armour
This armour will reduce the amount of damage you take from archer attacks.

Speed Scroll
This scroll will increase your speed stats proportionate to its level.
Found in boxes and dropped by officers
Survival Guide
When life is critical, attack strength will double upon getting up after falling.
Battle of Wu Zhang Plains - Shu Forces
Kill 2 of Wei's 5 Tiger Generals.
The Way of the Musou
This scroll allows you to perform your true musou attack regardless of whether or not your health is in the red zone.
Guan Yu's Escape - Shu Forces
Kill Xiahou Dun when he appears.
Tiger Amulet
This necklace of the White Tiger will increase your attack stats proportionate to its level.

Tortoise Amulet
This amulet of the Black Tortoise will increase your defense stats proportionate to its level.

Wind Scroll
This blue scroll increases your attack radius proportionate to its level.

Wing Boots
These boots of flight allow your character to jump higher than usual.

Campaign Against Sun Jian - Dong Zhou's Forces
When Sun Jian disappears, kill Lu Meng, Sun Shang Xiang, Sun Ce, Gan Ning and Zhou Tai.

Charge Bracer
This gauntlet-like piece allows the user to perform charge attacks uninterupted even while enemies are inflicting damage regardless of how powerful they are.
Campaign against Sun Jian - Yuan Shao's Forces
Enter the enemy castle before reinforcements arrive.
Dragon Amulet
This necklace of the Blue Dragon will increase your musou gauge stats proportionate to its level.

Start the stage riding on an elephant. Found in Nanman campaigns, the massive pachyderm is unparalleled in strength, yet its bulk drastically reduces speed.

Hex Mark Harness
Start the stage with Liu Bei's steed Hex Mark. This dark gray horse has been rumored to boost luck while riding the horse, otherwise has no unique effect.
Campaign against Liu Bei - Yellow Turban Force
Defeat Zhao Yun and Guan Yu before the back-up troops appear. Then go in Liu Bei's castle and fight his soliders until the item report appears.
Horned Helmet
Decorative helm that enables an increase in attack power when mounted on a steed (horse or elephant).

Huang Zhong's Bow
Imbued with the archer supremacy of its owner, this bow will increase your arrow attack damage.

Musou Armour
This armour allows a strenthened endurance to withstand arrow attacks unfaltered. You still recieve damage, but attack patterns are uninterrupted.
Musou Elixer
Sack of supplements that increases the speed of musou gauge recovery.

This decorative item of the Red Phoenix will increase your life gauge stats proportionate to its level.

Yellow Turban Fortress - Yellow Turban Forces
Kill Zhao Yun and Guan Yu.

Battle of Xia Pi - Allied Forces
Guan Yu must meed Red Hare down at the bottom island. No generals or subgenerals can be killed before obtaining the event.

A fancy belt that will increase your luck.

Storm Runner Harness
Start the stage on the Storm Runner Horse. This horse increases your attack while still mounted.
Campaign against Cao Cao - Lu Bu's Forces
Kill Xiahou Dun when he appears.

Battle at Xu Chang - Shu Forces
When you are presented with a choice of saving a seige weapon or the main camp, you must save the weapon as well as killing Xu Zhu.
